Pharmacist Medication Errors | Patient Safety On-Demand 1h

This course will demonstrate how to integrate safety principles into pharmacy practice to mitigate medication errors and enhance patient care outcomes. Learners will gain insights into fostering a culture of learning and reporting and distinguishing between a “just culture” and a “non-punitive” environment. Through real-life case scenarios, learners will explore the application of safe design principles in typical pharmacy settings to reduce patient harm while better understanding the benefits of patient safety organizations (PSOs).

Learning Objectives

Upon successful completion of this application-based CPE course, pharmacists should be able to:
1. Describe the principles of safe design that can reduce patient harm.
2. Discuss the importance of recognizing high-alert medications and strategies to minimize the risk of errors.
3. Explain how a culture of learning and a culture of reporting reduce patient harm.
4. List the benefits provided by patient safety organizations (PSOs).
5. Apply patient safety principles to address medication-related issues in real-life case scenarios.
