This course will explore the fundamental concepts of health equity and social determinants of health (SDoH) through an engaging discussion with leading professionals. Content will provide comprehensive insights into the newly introduced Health Equity Index (HEI) by the Center of Medicare and Medicaid Services (CMS), equipping pharmacists to identify and address health equity issues within community pharmacies. Learners will explore actionable strategies to mitigate disparities, enhance access, and improve outcomes, fostering a proactive approach to promoting health equity in this dynamic pharmacy education experience.

Upon successful completion of this knowledge-based CPE course, pharmacists should be able to:
1. Define health equity and social determinants of health (SDoH).
2. Recognize the impact of SDoH on health outcomes, patient care, and healthcare costs.
3. Explain the key elements of the new Health Equity Index (HEI) by the Center of Medicare and Medicaid Services (CMS).
4. Identify the role of community pharmacies in addressing health equity issues within the context of the upcoming HEI.
5. Describe strategies that address disparities in healthcare access, outcomes, and quality within a pharmacy setting.
