Patients visit pharmacies more than any other health care setting. This course discusses the need for Community Health Workers in pharmacy practice, and the services that can benefit patients and their healthcare outcomes.

Upon successful completion of this application-based CPE course, pharmacy technicians should be able to:
1. Differentiate between the roles of a pharmacy technician and community health worker.
2. Describe opportunities for pharmacy-based community health workers.
3. Assess community needs based on social determinants of health that could be addressed by community health workers.
4. Identify health education strategies to address health disparities in communities.
5. Describe reporting and documentation requirements for a community health worker during care coordination.
